How to see if you Qualify
Qualifying as a candidate for treatment is a simple and free process. It is done at no charge as a service to you, and to allow the doctor to evaluate your condition to see if you are qualified for our treatment program. The first step in the process is to call our office at the number below to get pre-qualified over the phone. Our staff will ask you a few questions about your condition(s) and history to begin the process. If you are pre-qualified at the end of the phone call, our staff will guide you through the rest of the qualification process.
